You can come into the surgery to view your medical record by prior appointment only at a time suitable to the practice.  Please phone or email thegoodpractice2@nhs.net to arrange this.

If you need paper copies of your notes for whatever reason you must request them in writing as we need  your signature, and we have 28 days to comply from receipt of the request. As of the 25th May 2018, all practices are required to provide patients with any data held by ourselves at no charge. We will always try to produce copies as fast as possible but are unable to offer an exact timeframe outside of the 28 day limit due to the variable size of patient records. If there are any mitigating circumstances please make us aware in your request so that they can be taken into account.
